Everyone has gone to a doctor before, but some reason they think the same rules, fees and relationships don”t apply to doctors of acupuncture and herbal medicine.  Let’s clear that up!

Communication Agreement-

This agreement outlines the terms and conditions regarding communication and services provided by our doctors.

  1. Purpose of Agreement

This agreement ensures effective communication and appropriate care by scheduling appointments for all patient inquiries, evaluations, and follow-ups.

  1. Scheduling Appointments

All questions related to acupuncture treatments, health concerns, or other topics requiring provider input must be addressed during scheduled appointments which are charged for. No advice, treatment recommendations will be provided outside of an appointment.

  1. Appointment Types

Patients may schedule the following types of appointments:  Each Type has separate fees.

– Consultation: Any new conditions to evaluate and discuss treatment options.

– Follow-Up Acupuncture Appointments: For ongoing care for a condition.

– Re-Evaluations: For assessing progress.

– General Questions and Advice: For inquiries requiring provider input, only if the provider determines the questions to be necessary and appropriate.

  1. Communication Policy

– Communication through phone will be limited to scheduling only.

– Clinical questions, treatment-related concerns, or personal health inquiries must be discussed during an appointment which has fees associated with it.

  1. Emergency Situations

In case of an emergency, patients should contact their primary care provider or local emergency services. This practice does not provide emergency medical services.
